Infection Risk and Aftercare *

I understand that infection is a possible risk associated with tattooing, especially if proper aftercare is not followed. I have received and agree to follow all provided aftercare instructions during the healing process.

Age and Consent Confirmation *

I confirm that I am over the age of eighteen and that receiving this tattoo is solely my decision. I consent to the tattoo procedure and to any reasonable actions by The Tattoo Truck's representative necessary to complete it.

Medical Disclosure *

If I have diabetes, epilepsy, hepatitis, hemophilia, HIV/AIDS, or any other communicable disease, heart condition, or take medication that thins the blood, I have informed my tattooer. I confirm that I am not pregnant or nursing, and I am not under the influence of drugs or alcohol.

Medical Disclaimer *

I fully understand that the tattoo artist is not a medical professional. Any advice or suggestions provided are not a substitute for professional medical guidance.

Skin Conditions *

I do not have any medical or skin conditions, including but not limited to acne, scarring (keloids), eczema, psoriasis, freckles, moles, or sunburn in the area to be tattooed, that could interfere with the procedure.

Allergic Reaction Disclaimer *

I acknowledge that it is not reasonably possible for The Tattoo Truck’s representatives to determine whether I may have an allergic reaction to pigments or processes used in my tattoo. I accept the risk that such a reaction may occur.

Effects of Future Skin Procedures *

I acknowledge that infection, scarring, migration, rejection, allergic reaction, and other complications are always possible as a result of receiving a piercing.

Color and Design Variability *

I acknowledge that it is not possible for The Tattoo Truck to determine whether I may develop an infection, and I accept the risk that such may occur.

Permanence and Representation *

I acknowledge that a tattoo is a permanent modification to my body and that no guarantees or representations have been made regarding the ability to later change or remove it. To the best of my knowledge, I have no physical, mental, or medical condition that would affect my decision or well-being related to this tattoo.
